راجعت دكتور عمود فقري وكتب التقرير التالي :

presenting complaints :
chronic neck pain, back pain, bilateral radiculopathy of upper and lower limbs for long time .
brief history :
this is 31-year-old male, with chronic neck pain, back pain, and bilateral radiculopathy of upper and lower limbs.
examination :
para-vertebral muscle spasm.
Muscle power 5/5 for both upper and lower limbs.
Decreased sensation over both upper and lower limbs.
Reflex is equivocal.
investigations :
MRI of cervical and lumbar spines.
diagnosis :
1- reverse cervical lordosis.
2- C4-C5 disc bulge.
3- mild disc protrusions of L4-L5 and L5-S1.
management :
pain treatment and muscle excrcise .
recommendations :
- pain treatment and physiotherapy.
-followup in the clinic.
