Patient History:
Adam is a 4 year old boy. He was adopted by his parents from Korea. He lives in the farm with
his grandparents. There were no conditions in the family history. The patient’s medical history:
rare colds, indigestion, a fractured finger 1 year ago.
Present complaint:
 Sudden fever 40ºC – started 5 days ago;
 Sore throat;
 Swollen lymph nodes in the cervical region
 Red tongue with swelling
 Rash in the abdomen and chest.
Adam’s grandmother was giving him traditional herbal medicines to reduce fever and sore
throat, but there was no apparent result.
Physical Examination and Preliminary Laboratory Results: The patient is given a
physical by the attending resident, and the following is noted:
 Fever 40ºC;
 Conjunctivitis without discharge;
 Rash: abdominal, thoracic area;
 Erythema of the oral mucosa and tongue redness and swelling as well.
 Pharyngitis;
 Cervical lymphadenitis;
 Erythema and edema of sole and palm
 Urinalysis: PH – 6, no RBC or leukocyte, the urine culture – negative. Blood test: WBC is 16,000/mm3
, RBC is 4,200 cells/ml , Hb 8.5 , culture – negative,
ESR – 100, CRP- 50
 BP – 120/70
 HR - 200
While the doctor was analyzing the lab results, the patient developed abdominal pain and
joint pain without swelling.
